Compassionate Care Manager

Our Care Management team at Sidney Health Center is dedicated to providing exceptional patient care. We are seeking a skilled and compassionate Registered Nurse or Licensed Practical Nurse to join our team as a Care Management RN or LPN.
As a Care Management RN or LPN, you will work closely with primary care clinicians and healthcare teams to develop, implement, and monitor personalized care plans for clients. You will serve as the primary point of contact for clients and their families, providing regular updates on care plans and addressing concerns.

- Collaborate with healthcare providers, social workers, etc., to ensure clients receive comprehensive and coordinated care
The ideal candidate will have graduated from an accredited school of nursing and hold a valid Montana Nursing License (RN or LPN) or current temporary license. Previous experience in caring for patients with chronic disease is highly desirable.
