  • Develop and implement individualized care plans to improve health outcomes and prevent hospitalizations.
  • Participate in regular routine consultations to monitor residents' health and provide timely interventions.

Requirements:

  • Vocational or General Registration with the Australian Health Practitioner Regulation Agency.
  • Full Australian work rights.
