About the Role

We are seeking a passionate and dedicated Community Support Worker to join our team in the Esperance area. As a Community Support Worker, you will provide one-on-one, individualised outreach support to people with a mental health diagnosis, helping them to establish routine and structure, and accessing relevant services.

Key Responsibilities
  • Provide individualised support to clients, helping them to establish routine and structure.
  • Assist clients in establishing and maintaining appropriate accommodation.
  • Coach and mentor clients in day-to-day life skills.
  • Actively participate in recreational, educational, and culturally appropriate supports.
  • Support clients to develop their support plans in partnership with key stakeholders.
  • Collaborate with clients and other supports to coordinate the delivery of services.
  • Monitor and maintain accurate case notes, up-to-date case plans, and files.
  • Observe and record client behaviour, needs, and interactions with others.
Requirements
  • Certificate IV in Community Services, Mental Health, or similar.
  • Unrestricted drivers' licence.
  • Access to a reliable car with minimum third-party property insurance.
  • Experience working with people with mental illness or psycho-social disability.
  • Passionate about client outcomes and mental health advocacy.
  • High computer literacy and administration skills.
  • Current First Aid Certificate.
