Support Coordination Lead

We are seeking a highly skilled and motivated leader to join our team as a Support Coordination Lead - Deaf Services. As a key member of our support coordination team, you will be responsible for overseeing the delivery of high-quality support coordination services to participants with complex needs.
About the Role- Lead a team of support coordinators, providing guidance and oversight to ensure high-quality service delivery.
- Develop and implement plans to promote community awareness of available services and support options.
- Collaborate with community organizations and service providers to develop and implement plans to track, monitor, and report participant progress against allocated budgets.
- Mentor and guide support coordinators to deliver high-quality services in line with participant needs.
- Facilitate referrals to internal and external services as required, ensuring participant needs are prioritized.
In your application, please outline your experience in senior or supervisory roles within the disability sector, as well as your skills in support coordination, case management, or a similar field.
We are looking for an individual who is passionate about provision of high-quality person-centred supports for Deaf and hard of hearing individuals, with a strong understanding of the Deaf community and culture.
