Job Title: Care Manager">

We are seeking a highly skilled and experienced Care Manager to join our team. The ideal candidate will have a strong clinical background, excellent leadership skills, and the ability to provide high-quality care to residents.

The Care Manager will be responsible for:

  • Ensuring the delivery of high-quality, person-centered care to all residents
  • Developing and implementing individualized care plans in collaboration with multidisciplinary teams
  • Conducting comprehensive assessments and investigating issues and incidents
  • Providing guidance and support to nursing staff on best practice and policy adherence

To be successful, you will have:

  • A minimum of 3 years post-registration experience as an RN
  • Experience as a Senior Registered Nurse or Clinical Nurse Leader
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
