**About This Opportunity**
Southern Adelaide Local Health Network is seeking a skilled Social Worker to join their Older Persons Mental Health Service.
This role works in a dual capacity between the Community and Inpatient Mental Health Teams, contributing to consumers' recovery and improved social functioning.
The successful candidate will be responsible for delivering clinical activities including mental health assessment, care planning, clinical review, psychosocial assessment, crisis management, clinical care coordination, advocacy, and specialised therapy programs.
Key responsibilities will include:
- Care planning, clinical review, psychosocial assessment, crisis management, clinical care coordination, advocacy, and specialised therapy programs.
- Advocating for and negotiating on behalf of the consumer with relevant groups/organisations.
- Undertaking health promotion activities such as developing and implementing educational services as required.
To succeed in this role you will require:
- An appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
- Ability to provide core social work skills including psychosocial assessment, counselling, individual and group work, relationship and family interventions, crisis intervention and brief therapy.
- Highly motivated and able to engage with consumers who have very high service needs.
