We are seeking a highly skilled and experienced Clinical Nurse Consultant to join our Aged Care Rapid Response Team. As part of this team, you will be responsible for providing expert clinical guidance and leadership to improve the care and support provided to older adults in Residential Aged Care Facilities and the community.
About the Role- This is a challenging and rewarding role that requires strong clinical skills, leadership abilities and a passion for improving healthcare outcomes for older adults.
- You will work closely with multidisciplinary teams to develop and implement comprehensive hospital avoidance strategies, fast-track diagnostic assessments and treatment plans to prevent hospital presentations or returns to residence.
- Develop and implement evidence-based practice guidelines to improve care and support for older adults.
- Foster strong relationships with aged care providers, health services and other stakeholders to ensure seamless transitions of care.
- Lead and coordinate clinical education and training programs for aged care staff to enhance their knowledge and skills in caring for older adults.
- Registered with Authority to Practice with Australian Health Practitioner Regulation Agency.
- Minimum five years full-time post-graduate experience with three years full-time equivalent in the specialty field.
- Approved post-graduate qualification (minimum Grad Diploma/Certificate level) or evidence of working towards Masters-level qualification.
