Mental Health Specialist Case Manager

This position is an exciting opportunity to work as a clinician in a community aged mental health team, providing psychiatric assessment and treatment.
You will visit clients at home, conducting intake and triage, offering crisis support, assessments, and case management services. This includes reviewing risk factors and providing guidance to reduce the impact of illness and enhance independence for clients and carers in various community and residential settings.
As a key member of the team, you will provide discipline-specific support and advice to ensure optimal outcomes for consumers.
Classifications for this role range from Occupational Therapist Grade 2 YB20 - YB23, reflecting a salary range of $88,030 - $101,197 per annum pro rata.
Key Responsibilities:
Holding a relevant degree and/or postgraduate qualifications
Being registered with AHPRA
Having 2 years of post-qualification clinical experience within Mental Health and/or aged care clinical services, including case management and care coordination experience
Demonstrating passion for achieving best clinical practice
Having knowledge of Mental Health and Wellbeing Act Victoria and other relevant legislation
Having a valid Nationally Coordinated Criminal History Check or willingness to obtain
