About the Role

We are seeking a skilled Case Manager to join our Intensive Therapeutic Care Team in Orange. As a Case Manager, you will provide high-quality support to children and young people residing in our therapeutic care homes.

Key Responsibilities
  • Develop and implement comprehensive therapeutic case plans that meet the individual needs and goals of children and young people.
  • Build and maintain functional relationships with children, young people, and their families to support positive outcomes.
  • Attend shifts within the intensive therapeutic care homes and work closely with the team to ensure seamless service delivery.
  • Meet with clients and family members to gather input for case plans and ensure their views are represented.
  • Coordinate and network with other agencies to effectively achieve case plan goals within tight time frames.
  • Participate in an on-call roster as required.
  • Ensure compliance with all internal policy and procedure, statutory and contractual requirements in relation to management of service delivery.
Requirements
  • Relevant bachelor's degree or a relevant diploma and be working towards a degree.
  • Previous case management experience within an Intensive Therapeutic Care or Out Of Home Care (OOHC) setting.
  • Experience supporting children and young people with disability, who have experienced backgrounds of trauma, abuse, and/or neglect.
  • Understanding of current casework practices for children and young people.
  • Understanding of trauma and its effects on children and young people.
  • Current driver's license.
