This role plays a crucial part in achieving our purpose by collaborating with children, young people, their families, and carers to create and implement case plans that aim to achieve permanency goals.
You will build strong relationships with both the children and their families, ensuring every voice is heard and every need is met.
- Key Responsibilities:
- Collaborate with children, young people, and their families to develop and implement case plans that promote permanency and well-being.
- Build and maintain strong relationships with children, young people, and their families to ensure their needs are met and their voices are heard.
- Provide support and guidance to children and young people who have experienced trauma, abuse, or neglect.
- Develop and maintain working understanding of relevant government policies and programs, including the NSW Permanency Support Program.
- Assess and address the needs of children and young people with disabilities, taking into account their individual circumstances and requirements.
Requirements:
- Bachelor's degree in Social Work, Social Welfare, Psychology, or related field
- Previous experience in case management within an Out-of-Home Care (OOHC) / Child Protection setting
- Experience supporting children and young people with disabilities, particularly those who have experienced trauma, abuse, or neglect
- Working understanding of the NSW Permanency Support Program and other relevant government policies
