Catholic Healthcare is seeking a highly skilled and compassionate Clinical Triage Delegate to join our Aged Care Assessment Services (ACAS) team. As a key member of our team, you will play a vital role in ensuring older Australians receive the appropriate care and support they need.

**Role Overview:**

The Clinical Triage Delegate is responsible for conducting thorough triage evaluations to identify the most suitable assessment pathway for clients. This involves establishing rapport with clients, gathering information relevant to their circumstances, and determining eligibility and assessment priority.

**Key Responsibilities:**

  • Conduct triage evaluations to determine client needs and develop an assessment plan
  • Determine eligibility and assessment priority for clients
  • Gather information relevant to client circumstances and confirm within referrals
  • Maintain currency of knowledge relating to the Aged Care Act and relevant frameworks and standards

**Requirements:**

  • Excellent communication and customer service skills
  • Excellent planning, organizing, and problem-solving skills
  • Effective negotiation, advocacy, and conflict resolution skills
  • Tertiary qualifications in a health-related discipline directly related to health, aged care, or a related specialist area
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or part of or eligible to be part of a relevant professional association
  • Completion of training requirements as specified under the My Aged Care Workforce Learning Strategy 2023
  • At least one year's aged care assessment experience preferred
