About the Role

We are seeking a highly skilled Registered Nurse to join our multidisciplinary healthcare team at Australian Unity. As a Clinical Care Coordinator, you will play a vital role in coordinating and supporting the delivery of home-based health care to clients across Australia.

Key Responsibilities
  • Coordinate and manage acute nursing Hospital Care at Home programs for clients discharging early from hospital to home-based care.
  • Support and communicate with clients remotely to achieve their personal and clinical health goals within their home environment.
  • Implement, monitor, and modify in-home care plans to meet client needs.
  • Manage clinical case escalations and variations to expected clinical progress.
  • Manage relationships with multiple stakeholders, including hospital referrers, surgeons/specialists, service providers, and funders.
  • Contribute to improving the health and wellbeing of hundreds of Australians.
Requirements
  • Qualification as a Registered Nurse with current AHPRA registration.
  • Experience with complex wound care and IV antibiotic management.
  • Experience working in a multidisciplinary collaborative team.
  • Excellent knowledge of hospital discharge planning.
  • Ability to work in a dynamic environment with tight timeframes and competing priorities.
  • Excellent clinical reasoning and problem-solving skills.
  • Excellent verbal and written communication skills.
  • Strong administration and time management skills, with proficiency in technology.
  • Passion for health and commitment to making a real difference in the community.
