Job Summary

We are seeking a highly skilled and compassionate Client Care Coordinator to join our team in Rockhampton. As a key member of our care team, you will be responsible for building strong relationships with clients and their families, assessing their needs, and developing personalized care plans to meet their unique requirements.

Key Responsibilities
  • Meet with clients and their families to understand their needs and preferences
  • Conduct thorough assessments to identify care requirements and develop comprehensive care plans
  • Provide high-quality care and support to clients in their homes, including nursing care and in-home services as needed
  • Manage client information and documentation, ensuring accuracy and compliance with regulatory requirements
  • Collaborate with care teams to ensure seamless delivery of care services
  • Communicate effectively with clients, families, and care teams to ensure excellent customer service
  • Stay up-to-date with industry developments, best practices, and regulatory requirements to maintain high standards of care
Requirements
  • EN/RN in community nursing with current AHPRA registration (preferred) or Cert III/Cert IV in Aged/Disability Care with minimum 4 years of experience
  • Proven experience in aged care, disability, or mental health services
  • Strong understanding of aged care and disability standards, consumer-directed care, and clinical governance
  • First Aid, current CPR, and National Police Check
  • WWC (or willingness to obtain) and driver's license with reliable car and comprehensive insurance
  • High level of computer literacy and excellent communication skills
Preferred Skills
  • Outgoing personality with high energy levels, good communication skills, and personable telephone manner
  • Ability to problem-solve and think quickly under pressure
  • Excellent networking, consultation skills to develop new referral sources and relationships with external organizations
  • Excellent customer service skills with exceptional verbal and written communication skills
  • Sound knowledge and understanding of home care services and guidelines
