About the Role

We are seeking a highly skilled and dedicated Senior Community Care Coordinator to join our team in Regional Queensland. As a key member of our community care services, you will play a vital role in shaping better lives for our clients and their families.

Key Responsibilities
  • Collaborate with clients, their families, and healthcare providers to create personalized care plans that focus on chronic disease management and well-being.
  • Develop and implement care coordination strategies to ensure high-quality, seamless, and client-focused care.
  • Work closely with multidisciplinary teams to manage complex health scenarios and provide exceptional care services.
  • Provide leadership and guidance to team members, promoting a culture of excellence and collaboration.
Requirements
  • A relevant Primary or Allied Health qualification (Registered Nurse preferred).
  • Proficiency in systems such as E Tools, My Aged Care, DEX Data Exchange, GeoOp, Medicare, & Proda.
  • A strong foundation in care coordination and experience with community aged care services.
  • Thorough knowledge of Aged Care and NDIS legislation.
  • Leadership skills that inspire teams, paired with outstanding communication and collaboration abilities.
  • Experience in working with complex health scenarios and managing multi-disciplinary teams.
  • Ability to juggle multiple priorities with ease.
  • Current Disability Services Positive Notice Card or Working with Children Check (or ability to obtain).
  • Valid Queensland Class "C" Driver's Licence.
  • National Criminal History Check and up-to-date vaccinations (or ability to obtain).
