Nurse Care Coordinator

We are seeking a highly skilled and compassionate Registered Nurse/Enrolled Nurse to join our team as a Care Coordinator at Right at Home Central Queensland. As a Care Coordinator, you will play a vital role in delivering high-quality, person-centered care to our clients in the community.
About the Role:This is a permanent part-time position, working under the supervision and support of a Care Manager. You will be responsible for:
- Providing exceptional care and support to clients in their homes, promoting independence and well-being.
- Collaborating with the Care Team to develop and implement individualized care plans.
- Ensuring compliance with relevant policies, procedures, and regulations.
- Communicating effectively with clients, families, and healthcare professionals.
- Participating in ongoing professional development to enhance your skills and knowledge.
To be successful in this role, you will need:
- Current registration with AHPRA.
- Satisfactory Australian National Police check.
- Current or willing to obtain a first aid and CPR certificate.
- Professional Indemnity Insurance.
- A smart mobile phone (iPhone/Android).
- Driver's licence and reliable presentable car.
- COVID-19 vaccinations.
