Nursing Care Manager

We are seeking a dedicated and compassionate healthcare professional to join our team as a Nursing Care Manager.
About the Role:As a key member of our healthcare team, you will play a vital role in managing and coordinating high-quality care for residents. Working closely with a supportive team of clinical staff, you will deliver person-centered care that reflects the unique needs and preferences of each individual.
Key Responsibilities:- Conduct comprehensive clinical assessments and develop tailored care plans to ensure optimal patient outcomes.
- Collaborate with residents and their families to understand their needs and preferences, ensuring that care is respectful and personalized.
- Lead and monitor care delivery to meet Aged Care Quality Standards, promoting a culture of excellence and continuous improvement.
- Mentor and support junior staff, including Enrolled Nurses, to enhance their skills and confidence.
- Facilitate training and development opportunities for team members to advance their knowledge and expertise.
- Ensure residents' physical, emotional, and social needs are met, including personal care, medication administration, nutrition, mobility, and comfort.
- A registered nurse with current AHPRA registration.
- Knowledge of the aged care sector and relevant standards an asset.
- Strong leadership and teamwork skills, with experience managing care teams.
- Experience in client assessments and developing person-centered care plans.
- Excellent communication skills – both verbal and written.
- A National Police Clearance (less than 6 months old).
- Up-to-date COVID and Flu Vaccinations highly regarded.
