We are seeking a highly skilled and experienced Care Coordinator to join our team at Help at Hand Support Services. As a Care Coordinator, you will play a critical role in ensuring the smooth running of our support services and providing exceptional care to our participants.
Key Responsibilities- Provide effective guidance and mentoring to Support Workers to ensure they have the necessary skills and knowledge to deliver high-quality care.
- Efficiently allocate and roster Support Workers to meet the needs of our participants, ensuring that their care plans are implemented and their goals are achieved.
- Process the onboarding and offboarding of staff and participants, ensuring a seamless transition and minimizing disruptions to our services.
- Develop and maintain effective relationships with participants and Support Workers, ensuring that their needs are met and their expectations are exceeded.
- Support the Participant's Liaison Manager in responding to customer feedback and complaints, ensuring that our services are delivered to the highest standard.
- Actively participate in the recruitment strategy, ongoing recruitment process, and orientations, ensuring that our team is equipped with the necessary skills and knowledge to deliver exceptional care.
- Promote and maintain a safe work environment in accordance with our Work Health & Safety policy, ensuring that our participants and staff are protected from harm.
To be successful in this role, you will demonstrate strong interpersonal skills, time management skills, attention to detail, and a conscientious work ethic. You will also possess:
- Experience and exposure to coordinating and allocating support services to people within their homes and community.
- Experience with rostering using a digital rostering software.
- Exceptional customer service skills, coupled with patience and commitment.
- The ability to build and maintain rapport with stakeholders.
- Possess a high level of computer literacy with experience using information management systems.
- Tertiary qualifications in human services, social science, or similar will be advantageous.
- Unrestricted working rights in Australia.
- The willingness to secure a Working with Children Check if required.
- University qualifications in a relevant field.
