Job Title:

A Clinical Care Specialist

  • We are seeking a skilled professional to provide person-centered quality clinical services to individuals with complex needs.
  • The ideal candidate will possess tertiary qualifications, current registration with AHPRA, and experience working with adults and/or children with disabilities.
Key Responsibilities:
  • Developing Comprehensive Care Plans:
  • Design and implement personalized health care plans for clients with complex requirements.
  • Nursing Care Services:
  • Deliver high-quality nursing care to individuals with urinary catheter, PEG, and bowel care needs.
  • Support Coordination:
  • Assist in coordinating clinical supports within established policies and guidelines.
  • Administrative Functions:
  • Ensure efficient administrative processes and provide regular feedback to the Service Manager and Clinical Lead.
  • Community Partnerships:
  • Build and maintain relationships with local community partnerships, including families and carers.
  • Diversity and Inclusion:
  • Promote a commitment to employment equity, diversity, workplace health and safety, risk management, and ethical practices.
Requirements:
  • Tertiary qualifications in a relevant field.
  • Current registration with AHPRA.
  • Experience working with adults and/or children with complex disabilities.
