Work with individuals and families at risk or experiencing homelessness in Brisbane City and South East Queensland.

We are seeking a values-driven professional to provide person-centred case management incorporating resilience, trauma-informed, and strengths-based practices.

  • Provide initial response and assessment of needs, risks, and vulnerabilities;
  • Establish relationships that encourage self-determination and participation;
  • Ongoing assessment and support to individuals in accordance with TSA case management models and standards of practice;

To be successful in this role you will need:

  • Tertiary qualification in Community Services, Social Work, or related field;
  • Relevant experience in a social service environment, including client assessment and case management;
  • Previous experience providing case management type support over the phone preferred;
  • QLD Working with Children Check;
  • Driver's Licence;
  • First Aid Certificate.
