About Home Care Nursing
">- This is a full-time opportunity for a qualified Registered Nurse to work in the field of home care, providing high-quality clinical nursing services to elderly clients in their own homes.
As a Community Registered Nurse, you will play a key role in assessing, planning and delivering personalised care plans that cater to the unique needs of our clients. You will work closely with a supportive multidisciplinary team, contributing to best-practice care planning and ensuring our clients can live independently, safely, and with dignity.
">Key Responsibilities:- Conduct thorough assessments of Home Care Package clients to identify their health and wellbeing needs.
- Develop individualised care plans that are evidence-based and tailored to each client's requirements.
- Provide high-quality clinical nursing care, including wound management, continence care, medication management, and promoting reablement.
- Collaborate with General Practitioners, allied health professionals, and family members to ensure holistic care is provided.
- Document all interactions, referrals, and clinical reporting accurately and efficiently.
- Support and guide Community Support Workers to align service delivery with care plans.
- Maintain accurate records and escalate any concerns or issues as required.
About the Ideal Candidate:
">- A current AHPRA-registered Registered Nurse with at least 2 years of clinical experience in aged care or a related field.
- Strong assessment and care planning skills, with the ability to think critically and make sound judgments.
- A current driver's licence and access to a reliable vehicle.
- The ability to travel throughout the Central Coast region as required.
- Excellent time management and interpersonal skills, with the ability to work independently and as part of a team.
- A commitment to delivering exceptional client-centred care and upholding professional standards.
