Job Description
  • This ongoing position offers an exciting chance to be part of a team committed to delivering high-quality healthcare services. Duties for the role will include:

The successful candidate will be involved in the assessment of referrals to Rehabilitation, GEM and Transition services from acute care, determining eligibility of clients, providing education to acute care staff, coordination of client services and continual monitoring of services provided by external and internal providers.

The ideal candidate will contribute to the reporting and quality monitoring mechanisms of the program, working effectively within a multidisciplinary team to achieve outstanding results.

  • Salary: $84,870 - $107,279 (Pro Rata) per annum.
About You
  • To be successful in this role, you will need to be a Registered or eligible Nurse with the Nursing and Midwifery Board of Australia, holding a current practicing certificate.
  • You should possess effective communication, problem-solving, conflict resolution, and negotiation skills, as well as the ability to work under minimal supervision.

The successful candidate will also have the ability to prioritize workload and meet set timelines, prioritizing tasks efficiently to achieve outstanding results.
