About the Role:

The HCP Care Manager is a key position within our organization, responsible for managing the clinical and ongoing care for recipients eligible for Home Care Packages funded by the Commonwealth Government.

Key Responsibilities:

  • Client Assessment and Care Planning: Provide comprehensive client assessment and development of care plans to support our clients at home.
  • Clinical Governance: Lead clinical governance and manage the quality management framework within Homecare services to ensure care standards are met.
  • Direct and Indirect Clinical Care: Provide direct and indirect clinical care to care recipients within scope of practice and ensure clinical needs are maintained.
  • Advanced Holistic Care: Provide advanced holistic care skills within a Case Management framework.
  • Program Development: Contribute to the ongoing development of the program including participating in development of procedures, planning and reviewing of care recipient services.
  • Reporting and Feedback: Meet on a regular basis with the HCP Team Leader to provide reports on achievement of KPIs and budget, and provide feedback to identify staff needs and positive achievements and further program development.

Requirements:

  • Qualifications: Bachelor of Nursing
  • Registration: APHRA registration essential
  • Experience: Demonstrated experience with home care packages, case management and excellent clinical knowledge
  • Knowledge: Knowledge of MAC and Aged Care funding streams
  • Skills: Demonstrated experience of community-based care and support services, highly developed assessment skills and the ability to creatively problem solve complex care situations, innovative and dynamic approach to the provision of care, demonstrated skills in networking, liaison and the development and maintenance of strong links with relevant service providers and users
  • Other: Ability to manage a budget effectively, work in a team environment and autonomously, valid working rights in Australia, police check (dated within the last 3 years), willing to obtain annual flu vaccinations, evidence of Covid 19 vaccinations, own vehicle/licence
