Dynamic Social Work Role

At Monash Health, we are looking for experienced Grade 2 Social Workers to join our subacute hospital setting. As a member of our dynamic and supportive team, you will have the opportunity to contribute to a positive team culture and work collaboratively with a multidisciplinary team.

This challenging role involves working with people from diverse backgrounds, completing complex psychosocial assessments, developing patient-centred care plans and managing complex discharges. You will also be required to work closely with internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient and community-based settings.

Required Skills and Qualifications
  • Bachelor Social Work or Master Social Work (Qualifying)
  • Experience working in a community health or hospital setting
  • Current driver's licence
  • Current Working with Children's Check
  • Eligible for AASW membership including continuing professional education
