Senior Community Care Nurse

As a compassionate and respectful nurse, you will be part of our team delivering clinical in-home services including care planning, medication management, wound care, chronic disease management, palliative care, and diabetes management. You will also supervise non-clinical team members in the community and handle intake, care planning, and reporting.
The successful candidate will possess excellent communication skills and have the ability to work independently and in teams. They must have strong time management and organizational skills as well as a commitment to ongoing professional development.
Job Requirements- Qualified Enrolled Nurse
- Current AHPRA registration
- First Aid & CPR certification
- Australian Police Check
- Professional Indemnity Insurance
- Experience in community care assessment and planning
- Valid driver's license and reliable vehicle
- Smart mobile device (iPhone/Android)
- Passion for high-quality, client-focused community care
- Cert IV in Training and Assessment (highly desirable)
- Strong communication skills
- Ability to work independently and in teams
- Excellent time management and organizational skills
- Commitment to ongoing professional development
- Residing in Western Australia is required
