
Community Mental Health Specialist

The FACT team is responsible for providing ongoing recovery-oriented assessment and treatment through case management, aimed at improving the quality of life for individuals with severe and complex mental health needs requiring intensive intervention in a community setting.
Key Responsibilities:- To deliver multidisciplinary, recovery-oriented assessment and treatment through case management for individuals with severe and complex mental health needs in a community setting.
- Work collaboratively with clients to identify and achieve their goals and objectives.
- Develop and implement individualized treatment plans that are tailored to meet the unique needs of each client.
- Provide education and support to clients and their families to help them better understand and manage their mental health conditions.
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services.
- Document client progress and outcomes to inform future treatment decisions.

- A Bachelor of Social Work or related field qualification.
