Job Overview

The FACT team is responsible for providing ongoing recovery-oriented assessment and treatment through case management, aimed at improving the quality of life for individuals with severe and complex mental health needs requiring intensive intervention in a community setting.

Key Responsibilities:
  • To deliver multidisciplinary, recovery-oriented assessment and treatment through case management for individuals with severe and complex mental health needs in a community setting.
What You Will Do:
  • Work collaboratively with clients to identify and achieve their goals and objectives.
  • Develop and implement individualized treatment plans that are tailored to meet the unique needs of each client.
  • Provide education and support to clients and their families to help them better understand and manage their mental health conditions.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services.
  • Document client progress and outcomes to inform future treatment decisions.

Requirements:
  • A Bachelor of Social Work or related field qualification.
  • Current registration with the relevant health practitioner regulatory agency.
  • Experience working with individuals with complex mental health needs.
