We are seeking a skilled professional to fill the role of Client Liaison Coordinator.

About the Role:

The primary responsibilities of this position include connecting clients with internal and external services, guiding them through their journey, and helping them achieve their goals. This may involve developing service agreements, addressing client feedback, monitoring budgets, conducting home visits, and ensuring compliance with relevant standards.

Key Responsibilities:
  • Develop and implement effective service plans.
  • Address client concerns and assess referrals to ensure eligibility.
  • Monitor client finances and costings.
  • Conduct home visits to understand client needs.
  • Maintain high standards of compliance.
Requirements:
  • Relevant qualifications or experience in disability services or aged care.
  • Excellent interpersonal and communication skills.

  • Ability to work independently and as part of a team.
  • Organizational skills to manage workload and meet deadlines.
