Care Transition Specialist

About the Role:

The Care Transition Specialist will provide tailored support services to young adults transitioning out of care. This role requires effective engagement with clients, their families, and support networks to identify needs and aspirations.

The specialist will develop and implement a personalized care plan in collaboration with the client, incorporating financial supports where applicable.

Key Responsibilities:
  • Maintain accurate and detailed records of client interactions.
  • Deliver services and supports that align with the program model of care.
  • Provide risk management and crisis response for clients experiencing vulnerability or trauma.
  • Engage with allocated client cases, ensuring timely outreach and consent.
  • Develop clear goals and care plans based on client-led assessments.
  • Facilitate one-to-one support in securing employment, education, or training pathways.
  • Participate in team building opportunities as directed by Manager.
Requirements:

This role requires a current Cert IV in Social Work, Human Services, or a related field, along with at least 12 months of experience in case management. Alternatively, a graduate degree in a relevant field with a minimum 10-week full-time placement in social services is acceptable.

Candidates must demonstrate ability to work in a performance-driven environment, meeting individual targets. Experience with high-risk and crisis clients, including planning and organisational skills, is essential.

A current driver's licence, Working with Children's Card, and flexibility to work outside standard business hours are required.
