We are seeking an experienced occupational therapist to join our home and community care team.
You will work across our home care and transition care program (TCP), supporting older adults to regain their independence after a hospital stay. This role sits within our allied health team and forms part of our wider multidisciplinary team.
Your key responsibilities include conducting client assessments, delivering tailored therapy, prescribing home modifications, and collaborating with multidisciplinary teams.
