Job Title: Occupational Therapist

About the Role:

We are currently seeking Occupational Therapists to join our multidisciplinary team. You will work across our Home and Community Care program, supporting older adults to regain their independence after a hospital stay.

This role involves conducting client assessments to determine capabilities goals and support needs, delivering tailored goal-oriented therapy, prescribing home modifications aids and equipment as required, providing guidance to therapy assistants, communicating effectively with clients families and multidisciplinary teams, collaborating across the organization to ensure high-quality care delivery, contributing to quality improvement and best practice initiatives, and working across various funding streams.

Key Responsibilities:

  • Conduct thorough assessments of clients' capabilities goals and support needs
  • Develop and deliver tailored therapy plans to promote independence
  • Prescribe necessary home modifications aids and equipment
  • Provide support and guidance to therapy assistants
  • Maintain effective communication with clients families and multidisciplinary teams
  • Collaborate with the team to ensure high-quality care delivery
  • Contribute to ongoing quality improvement and best practice initiatives
  • Work across multiple funding streams

Requirements:

  • Current registration as an Occupational Therapist with AHPRA is essential
  • A valid Australian driver's license and reliable vehicle are also essential
  • Previous experience in aged care community health or rehabilitation settings is desirable
  • Demonstrated ability to conduct thorough assessments and develop person-centered therapy plans
  • Experience working with clients with dementia chronic illness or mobility limitations is highly regarded
  • Knowledge of aged care quality standards funding models and clinical documentation is also valuable
  • Ability to work independently and collaboratively within a team
  • Effective time management and organizational skills to manage a varied caseload
  • Strong interpersonal and communication skills to engage respectfully with older clients families and other stakeholders
