Senior Home Care Support Worker

As a Senior Home Care Support Worker, you will be responsible for providing high-level care to older people and their families in the community. Your role will involve visiting clients in their homes, supporting personal care needs, assisting with light domestic tasks, facilitating social support, and helping clients attend important appointments.

This is an excellent opportunity to work in a collaborative team environment and develop your skills in home care support. You will have the opportunity to work closely with other healthcare professionals and learn from them.

The ideal candidate will have strong communication and interpersonal skills, as well as experience working with older adults. You should also have a current authority to practice as an Enrolled Nurse and a valid NSW driver's licence.


Key Responsibilities:
  • Deliver high-quality home care support under the TACP framework, including personal care assistance and light domestic duties.
  • Facilitate social support and assist clients with appointments and transport needs as appropriate.
  • Collaborate with the Community Nursing team to provide ongoing care and professional development opportunities.
  • Assess client needs, recognize changes in condition, respond to emergencies, and coordinate with other health services.
  • Travel to client homes; maintain a valid NSW driver's licence and willingness to travel across the community.

Required Skills and Qualifications:
  • Current Authority to Practice as an Enrolled Nurse with AHPRA.
  • Current NSW Driver's Licence and willingness to travel.
