Transitional Aged Care Program - Community Enrolled Nurse

Narrabri Community Health Service | Permanent Part Time, 31 hours per week | Enrolled Nurse

Overview

Join our Transitional Aged Care Program (TACP) and Community Nursing teams to provide high-level care to older people and their families in the Narrabri community. You will visit clients in their homes, support personal care needs, assist with light domestic tasks, facilitate social support, and help clients attend important appointments. You will also have opportunities to work with the Community Nursing team to learn and grow.

Responsibilities
  • Deliver in-home support under the TACP framework, including personal care assistance and light domestic duties.
  • Facilitate social support and assist clients with appointments and transport needs as appropriate.
  • Collaborate with the Community Nursing team to provide ongoing care and professional development opportunities.
  • Assess client needs, recognise changes in condition, respond to emergencies, and coordinate with other health services.
  • Travel to client homes; maintain a NSW driver's licence and willingness to travel across the community.
Qualifications
  • Current Authority to Practice as an Enrolled Nurse with AHPRA
  • Current NSW Driver's Licence and willingness to travel
