Family Transition Liaison

Our organization is seeking a dedicated Family Support Coordinator to play a pivotal role in supporting children and families during their transition from early childhood services into primary school.
The ideal candidate will be responsible for removing barriers to education by connecting families with essential services, promoting positive attendance, behavior, and learning outcomes.
Key Responsibilities:- Collaborate with external partners to drive long-term solutions for families and children.
- Work closely with teachers and support staff to identify child needs and connect families with relevant services.
- Establish strong relationships and partnerships with students, parents, school staff, government agencies, and community services.
- Provide expert input on decision-making processes to ensure optimal outcomes for children and families.
