Youth Transition Support Specialist

This role provides direct support to young people with complex needs, most at risk of homelessness as they transition to adulthood and post-care.

Maintain sound, accountable and legislatively compliant records in relation to your work practices and job activities. We foster a culture of continuous quality improvement and alignment with regional and programmatic priorities.
Key Responsibilities:We are seeking an experienced professional to consult and develop relationships with government and non-government agencies including Queensland Health, Youth Justice, Office of the Public Guardian, Department of Housing and Public Works, and the National Disability Insurance Agency to ensure efficient, effective and financially supported service delivery.
Collaborate with regional line manager and senior program officer within a hub and spoke model ensuring continuous quality improvement and alignment with regional and programmatic priorities and identify opportunities for outcome improvement when supporting young people transitioning from care.
