About the Role
  • We seek an exceptional Aftercare Coach with relationship-building and advocacy skills to walk alongside young people and support their personal growth.
  • The ideal candidate will have experience working with young people, relevant tertiary qualifications in social science or equivalent, and a current driver's licence.
  • The role involves delivering intensive aftercare support, including case management, goal planning, and service coordination, as well as facilitating access to entitlements and assistance.
  • You will work closely with young people to develop independent living skills, strengthen connections to family, culture, and community, and deliver holistic, personalised mentoring and group sessions on topics including social skills, job readiness, financial literacy, and health and wellbeing.
