About the Role

We are seeking a highly skilled Mental Health Clinician to join our team as a Community Care Specialist. In this role, you will be responsible for delivering direct treatment and coordinating care for clients with severe mental health conditions.

Key Responsibilities:
  • Conduct thorough clinical assessments and develop tailored treatment plans
  • Deliver evidence-based psychosocial interventions
  • Manage a clinical caseload and conduct risk assessment with management plans
  • Engage in data collection and contribute to monitoring and evaluation
  • Collaborate with multidisciplinary teams and external stakeholders
  • Facilitate care planning and review processes
  • Monitor and escalate mental health needs as necessary
Requirements:
  • Current annual practicing certificate from AHPRA or registered with AASW
  • Tertiary qualification in psychology, social work, mental health nursing, or a relevant discipline
  • Experience with complex mental health cases
  • Experience working in cross-cultural settings, particularly with First Nations people
  • Ability to work in a team within a diverse environment
  • Strong problem-solving and analytical skills
  • Excellent organisational and time management skills
  • Excellent verbal and written communication skills
  • Proficiency with Microsoft Office and software
