We are seeking a dedicated and organized professional to join our team as a Compassionate Care Manager.
This role is perfect for someone who wants to make a positive impact in the lives of others. As a Compassionate Care Manager, you will be the primary point of contact for clients and their families, providing support and guidance to ensure they receive the best possible care.
- Main Responsibilities:
- Conduct comprehensive client assessments and develop personalized care plans to meet their unique needs.
- Build strong relationships with clients and their families, addressing their concerns and ensuring their satisfaction with our services.
- Collaborate with our team of home care workers to provide professional health and clinical advice and direction.
- Maintain accurate and up-to-date client records and information.
Key Requirements:
- Formal tertiary qualification in nursing (e.g., Bachelor of Nursing).
- Current registration as a Registered Nurse with the relevant authority.
- Demonstrated knowledge and experience in the aged care sector.
- National criminal history/police check.
- Working with Children Check.
- Valid First Aid and CPR certificate.
