About the Role

The Northern Adelaide Local Health Network is seeking a skilled Transition Care Program Coordinator to join the Aged Care, Rehabilitation and Palliative Care Division.

As a Transition Care Program Coordinator, you will play a vital role in supporting patients and their families during the critical transition period between hospital and home or residential aged care.

Your responsibilities will include coordinating and facilitating the efficient discharge of eligible clients, ensuring they receive the necessary support and services to continue their recovery journey in a comfortable and familiar environment.

Key Responsibilities
  • Coordinate and facilitate the efficient discharge of eligible clients
  • Ensure clients receive necessary support and services
  • Collaborate with multidisciplinary teams, clients, and their families
  • Contribute to the continuous improvement of the Transition Care Program
Requirements

To be eligible for this role, you must hold an appropriate degree or equivalent qualification for one of the following Allied Health disciplines working in the service:

  • Occupational Therapist
  • Physiotherapist
  • Speech Pathologist
  • Exercise Physiologist
  • Australian Association of Social Workers
  • Pharmacist
