Our Homelessness Stream offers high-quality services to people at risk or experiencing homelessness. We support adults and families, including accompanying children, with accommodation, case management, outreach support, financial assistance, connection, and referral to specialist services.

About the Role

We are seeking a values-driven Housing Support Specialist who will provide person-centered case management support in a community setting. This involves assisting individuals to achieve housing stability, improved social inclusion, and self-mastery, enabling them to sustain tenancy and lead their best lives.

This support is achieved through incorporating person-centered, recovery, trauma-informed, and strengths-based practices while working effectively with the team to provide a comprehensive service that reflects our vision, mission, and values.

You will Successfully

  • Establish relationships with individuals encouraging self-determination and participation.
  • Provide case management support in accordance with TSA models and standards of practice.
  • Complete thorough assessments, inclusive of needs, risks, and vulnerabilities.
  • Ongoing assessment of presenting issues, vulnerabilities, and needs from initial intake to exit.
  • Individual case planning in collaboration with the individual, including interventions addressing needs or goals and barriers to achieving those.

About You

  • Tertiary qualification in Cert IV - Community Services, Social Work, or related field.
  • A degree-level qualification is preferred.
  • Relevant experience in a social service environment, including client assessment and case management.
  • QLD Working with Children Check.
  • Driver's Licence.
  • First Aid Certificate.

Salary and Conditions

This is a max-term full-time position contracted until September 2025, located in Spring Hill, QLD. Salary and conditions are in accordance with the Social, Community, Home Care, and Disability Services Award.
