About the Role

As a Social Services Case Manager, you will work with individuals and families in need, providing initial assessment, appropriate referral, and person-led case management to achieve set outcomes.

You will establish relationships that encourage self-determination and participation, provide case management support in accordance with TSA standards of practice, and develop individual case plans in collaboration with clients.

The role involves working effectively with other team members to provide a comprehensive service that reflects the vision, mission, and values of The Salvation Army.

This is a permanent full-time role based at Spring Hill, Brisbane City.

Key Responsibilities

  • Answer calls in a timely manner, providing initial response and assessment, inclusive of needs, risks, and vulnerabilities.
  • Respond to Walk-ins on site and provide assessment and referral as appropriate.
  • Establish relationships with individuals who access the service, that encourage self-determination and participation.
  • Provide case management support to individuals in accordance with TSA case management models and standards of practice.
  • Develop individual case plans in collaboration with clients and include a range of interventions that address the needs or goals of clients, as well as barriers to achieving those.
