Job Description

We are seeking a skilled professional to join our team as a Care Manager.

As a Care Manager, you will play a vital role in delivering high-quality telephone-based client care for elderly Australians.

You will be responsible for delivering excellent customer service and aged care management skills to make a positive impact on the community.

Key responsibilities include:

  • Delivering exceptional customer service and aged care management skills
  • Developing, monitoring, and reassessing care plans in consultation with clients, their families, and other healthcare professionals
  • Working collaboratively with our Nurses, Regional Managers, and other teams to facilitate, develop, and shape our service offerings
  • Monitoring client service delivery across all levels under the government-funded CHSP and homecare packages programs
  • Engaging with internal teams to facilitate client on-boarding
  • Maintaining compliance with all relevant legislative and industry standards
Required Skills and Qualifications

Excellent communication skills are essential for this role.

Experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards is necessary.

Strong computer skills in CRM systems, database management, and Microsoft Office are required.

The right to work in Australia is also necessary.

National Police Clearance or willingness to obtain (we will organize via WorkPro at no cost) is required.

Proof of Covid Vaccination is mandatory.
