Job Overview

We are seeking a qualified and reliable Care Coordinator - EN/RN to join our team. As a Care Coordinator, you will be responsible for meeting with clients and their families, assessing their needs, and building strong connections. You will also conduct in-home client care assessments and prepare care plans to best meet client requirements.

Main Responsibilities

  • Assess client needs and build strong relationships with clients and their families
  • Conduct in-home client care assessments and create comprehensive care plans
  • Provide some nursing care and in-home care services as needed
  • Handle information requests, feedback, and general enquiries
  • Schedule services for clients by rostering care workers based on qualifications and availability
  • Ensure accurate and comprehensive documentation is completed in our databases and software programs

Requirements

  • A current AHPRA Registration
  • Qualifications in Nursing
  • Highly developed verbal and written communication skills

  • Current or willing to obtain Australian National Police Check & Working with Children Check
  • Current or willing to obtain First Aid & CPR certificate
  • Full Driver's license and your own reliable, insured vehicle
