The Opportunity

As a Clinical Case Manager, you will play a crucial role in coordinating tailored care plans for our clients. You will be the key point of clinical support, ensuring that care delivery aligns with industry standards and best practices.

  • Conduct comprehensive client assessments and risk reviews in home settings
  • Develop, review, and manage personalized care plans in line with funding requirements
  • Provide clinical oversight and mentorship to care staff
  • Lead case conferences, care reviews, and service plan updates
  • Liaise with healthcare professionals and families to coordinate multidisciplinary input
  • Deliver nursing care in the community, including wound management and clinical monitoring
  • Monitor documentation for accuracy and quality compliance
Required Skills and Qualifications

To excel in this role, you must possess:

  • Registration as a Registered Nurse with AHPRA (unrestricted)
  • 2+ years of experience in case management for Home Care Packages
  • Proven understanding of aged care legislations and care planning processes
  • Strong interpersonal skills and ability to support staff and clients with empathy and professionalism
  • Competence with electronic documentation systems and intermediate computer skills
