The Private Client Care Specialist will serve as an advocate for our 24/7 clients, facilitating customized care to meet their requirements. Key responsibilities include:

  • Comprehensive client assessments
  • Developing personalized care plans
  • Case management and monitoring client wellbeing
  • Collaborating with stakeholders to manage changing care needs

This position requires representing the company as a preferred service provider, necessitating knowledge of our services.

To excel in this role, you should possess:

  • At least 2-3 years' experience in case management
  • Excellent assessment, documentation, and implementation skills
  • Experience working with complex care needs, including dementia
  • Strong communication, negotiation, and interpersonal skills

Pursuing a tertiary degree in health and/or social work is highly desirable.
