Job Role: Clinical Aged Care Needs Assessor
About the RoleWe are seeking an experienced and passionate clinical professional to join our team as a Clinical Aged Care Needs Assessor. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care services to older people in our district.
Your expertise in aged care assessment and your ability to communicate effectively with patients, families, and carers will be invaluable in ensuring that our clients receive comprehensive and person-centered care.
Key Responsibilities- Conduct thorough assessments of older people's needs and develop individualized care plans.
- Collaborate with multidisciplinary teams to ensure seamless delivery of care services.
- Provide education and support to patients, families, and carers on various aspects of aged care.
- Tertiary qualifications in a relevant clinical discipline (nursing, occupational therapy, social work, physiotherapy) with full registration or eligibility for membership of the relevant Australian peak professional body.
- Proven high standard of written and verbal communication skills.
- Demonstrated experience working in aged care settings with a strong commitment to delivering quality care.
