Job Overview

This role is focused on supporting carers through the Carer Support Planning Process. It involves an intake, registration and needs assessment, support planning, coordination, support and mentoring to ensure carers are holistically supported.

  • Supporting carers in need of guidance and assistance
  • Assessing carers' requirements and developing personalized plans
  • Collaborating with various stakeholders to ensure comprehensive care

The ideal candidate will possess a strong understanding of social work principles and practices, as well as excellent communication and interpersonal skills. They will be able to effectively communicate with people from diverse backgrounds and provide empathetic support.

This position requires strong organizational and administrative skills, as well as experience in service delivery and telephone support work.

The role is available full-time or part-time and can be based in our offices in Newcastle or Coffs Harbour.
