
Navigating Care Pathways Specialist

The Aboriginal Health Navigations Specialist is responsible for facilitating access to healthcare services and resources for children and young people in out-of-home care.
This role requires a strong understanding of local Aboriginal culture and community, as well as the ability to work effectively with multiple external services. The successful candidate will have excellent communication and interpersonal skills, and be able to provide sensitive and holistic support to clients and their families.
- Identify and address barriers to accessing healthcare services
- Liaise with dental services, maternal child health nurses, and other relevant stakeholders
- Attend multidisciplinary assessments and make referrals for children and young people in care
The ideal candidate will be motivated, self-motivated and have a passion for making a positive difference in the lives of vulnerable children and young people.
