Job Details

Right at Home is one of the world's largest home care providers, with approximately 600 offices in eight countries.

This role requires flexibility, as some weekend supports may be required. This time will be offset as reasonably as possible through the week.

The key responsibilities include:

  • Care Assessments and Planning
  • Development of Government-Compliant Care Management Plans
  • Outcome-driven Care Planning and Implementation
  • Operation of the Care Planning and other management systems
  • Personal care supports and services
  • Quality assurance and improvement

Essential Qualifications:

  • Minimum of Diploma in Nursing or relevant experience in community services
  • Current First Aid & CPR certification; and valid Driver's License

Desirable Skills:

  • Good understanding of Government funding environments for aged care (primarily CDC) & NDIS
  • Knowledge of hospital discharge procedures
  • Strong teamwork, caring, and quality service ethos
  • Proven leadership abilities, professionalism, good reputation, and respect for staff, clients, and their families
  • Good judgment, quick decision-making, excellent communication, staff management, and administrative skills
  • Current National Police Clearance
