Our service is seeking a dedicated professional to provide high level Social Work services to consumers, carers and families. This role offers the opportunity to make a real difference in people's lives while working within a dynamic and supportive team.
**Key Responsibilities:**
- Provide assessment, counselling, crisis management, case management, advocacy, intervention, appropriate referral and discharge planning services to consumers, carers and families.
- Demonstrate skills and knowledge in the provision of services to consumers, carers and families requiring assistance with health outcomes.
- Prioritise and manage workload demands, meet deadlines and complete activities within scope of practice.
