We are seeking an experienced Allied Health Professional to join our multidisciplinary team and provide high-quality clinical coordination services for young people with complex care needs.
This is an exciting opportunity to work in a dynamic environment, delivering person-centered mental health services to children, young people, and their families/carers. As a Clinical Coordinator, you will be responsible for facilitating identified needs, undertaking high-level assessments, and developing individual plans that bring together the right mix of services across government and non-government agencies.
Key Responsibilities:- Work collaboratively with a diverse multidisciplinary team to provide specialist clinical coordination services that support right place, right care principles.
- Undertake high-level assessments and develop individual plans that bring together the right mix of services across government and non-government agencies.
- Promote a positive working culture by developing effective partnerships and strong working relationships with all stakeholders within the Agency and with external community service providers.
