As a senior care manager, you will play a pivotal role in our community by crafting and implementing tailored care plans for elderly clients receiving in-home care.

Duties:

  • Managing Home Care Packages for eligible elderly clients accessing government funding
  • Visiting prospective clients to facilitate the onboarding process and reviewing existing clients' plans
  • Coordinating client interactions to ensure seamless support that meets the unique needs and best interests of each client

Essential Skills & Qualifications:

  • Proven experience in Aged Care or Home Care Package Management
  • Certificate IV in Community Services
  • Excellent time management and administrative skills
  • Effective communication and customer service skills
